COVID-19 Supplies Request Form

Each school, college or department should follow the guidelines to strategically compile their initial COVID-19 supply needs in preparation for campus re-opening in August. Once compiled, they should submit a single COVID-19 Supply Request Form to request what is needed initially. Departments must not request supplies to stockpile future use. Additional or replenished supplies will be available when needed.
