Application and Forms

The use of any biological materials requires prior review by EHSS. Research involving materials classified as biosafety level 2 (BSL-2) or higher, human materials, and/or recombinant or synthetic nucleic acids covered under the NIH Guidelines, must be reviewed and approved by the University’s Institutional Biosafety Committee (IBC) prior to initiation of experiments. Please complete the Biohazardous Material Use Application prior to working with any biological materials.

A biosafety SOP is required for research involving materials classified as BSL-2 or higher. A Biosafety SOP Template is available for use and must be submitted for approval with the Biohazardous Material Use Application.

Laboratories using biological materials must meet the requirements outlined in the University’s Biohazardous Laboratory Design Criteria.

Principal Investigators (PIs) must notify EHSS of any modifications to biohazardous material use before it occurs, and provide an annual biological inventory update to ensure  information is complete and accurate.

All use of biohazardous materials must comply with the University Biosafety Program. For questions or support in completing the Biohazardous Material Use Application or Biosafety SOP Template, contact EHSS at ehss@syr.edu.
