The University requires that all protocols involving the use of biohazardous material be registered with the Institutional Biosafety Committee (IBC). Please complete the institutional biosafety committee protocol application form [PDF] to submit biological research information to the the IBC.
Laboratories working with potentially biohazardous material must maintain a lab specific Standard Operating Procedure (SOP) for the control of biohazards. The Biosafety SOP Template [PDF] can be modified and submitted for approval with the IBC application.
On an annual basis, Principal Investigators must provide a Biological Inventory Update to ensure that the information the University has on record for each research laboratory is complete and accurate. The biological research information and inventory registration/update form should be used to submit biological research information to the the IBC. By entering your NetID and password, you will have access to any information/inventory you may have previously submitted and have an opportunity to provide updated information.
Modifications to IBC approved protocols may be requested by completing a IBC protocol amendment form [PDF]. Dependent upon the extent of the proposed modification(s), the IBC may request that the PI submit a new application for approval.
