Contact the Department of Public Safety at 315-443-2224 for immediate assistance. DPS will dispatch the appropriate emergency responders.
Notify DPS immediately in the event of a fire, serious injury, University vehicle accident or hazardous material incident.
Reporting an Incident
Personal injuries, vehicle accidents, property damage and hazardous material incidents and near-misses at the University should reporting using the University’s Origami Risk Incident Reporting System. You will be prompted to input details about the incident, including the date, time, and a description of the incident.
To access Origami, you can either:
- Directly visit the Origami Portal
- Navigate to the Employee Resources tile on MySlice, then click “Report an Incident.”
Reported incidents will be investigated by the appropriate University departments, including Risk Management and Emergency and Environmental Risk Services, to examine the root cause and implement corrective actions as needed.
When in doubt as to whether or not you should report the incident, err on the side of caution and report.
