Incident Reporting

Syracuse University uses Origami Risk to manage incident reporting.

All faculty and staff members should utilize Origami, which replaces all paper-based processes for reporting accidents, injuries, and near-misses on campus. It will allow several University departments, including Risk Management and Emergency and Environmental Risk Services, to examine the root causes of accidents, address unsafe areas of campus, and, if needed, make changes to the campus landscape (repair cracked sidewalks, address loose pavers or bricks, improve sight lines for bikers and pedestrians).

To access Origami, you can either directly visit the portal, or navigate to the Employee Resources tile on MySlice, then click “Report an Incident.” You will be asked to input details about the incident, including the date, time, and a description of the incident.

Examples of incidents that should be reported include:

  • Student or visitor injuries
  • Workplace injuries
  • Property damage
  • Any other incidents requiring review by Risk Management or Environmental Health and Safety Services

When in doubt as to whether or not you should report the incident, err on the side of caution and report.
