- For general pesticide applications, please submit the form at least twenty-four (24) hours prior to the application.
- For pesticide applications at or within 150 feet of a child care facility, please submit the form at least seventy-two (72) hours prior to the application.
- For agricultural pesticide applications (e.g., greenhouse, growth rooms, etc) please submit the form at least seventy-two (72) hours prior to the application.
- A copy of the Product Label and Safety Data Sheet for each pesticide applied at the University must be submitted to EHSS for recordkeeping.
