Employees who have been identified as having reasonably anticipated exposure to blood or other potentially infectious materials during their course of employment at the University are offered the Hepatitis B vaccine, at no cost to the employee. The vaccine is offered annually to employees during bloodborne pathogens training. Employees will be directed to complete a Hepatitis B Vaccine Declaration form where they can opt to receive or decline the Hepatitis B Vaccine.
Employees who decline vaccination still have the option to request the vaccine series if they change their mind. This request can be made at any time by submitting an updated Hepatitis B Vaccine Declaration form.
