Hepatitis B Vaccine for Affected Employees

Employees who have been identified as having reasonably anticipated exposure to blood or other potentially infectious materials during their course of employment at the University will be offered the Hepatitis B vaccine, at no cost to the employee.  The vaccine will be offered to employees as part of the bloodborne pathogen training. Employees will be directed to complete a Hepatitis B Vaccine Declaration form where they can opt to receive (consent) or decline the Hepatitis B Vaccine.

Employees who consent to the Hepatitis B Vaccine will be directed to contact the University’s Health Center to schedule an appointment to start the vaccine series.

Employees who decline the vaccine still have the option to request the vaccine series if they change their mind.  This request can be made at any time by submitting the Hepatitis B Vaccine Declaration form available on EHSS’ website.
