Laboratory Incident Reporting

Notify the Department of Public Safety (DPS) at 315-443-2224  when incidents and emergencies occur on campus,  including in laboratories. DPS will notify and dispatch appropriate emergency responders.

Serious Lab Incidents 

For serious laboratory incidents, including those involving fire, explosion, personal injury, property damage, or hazardous materials release, immediately notify DPS, and follow the University’s serious laboratory incident notification process.

Post Incidents and Near Misses Reports

Laboratory personnel should complete a Laboratory Incident and Near Miss Report to record laboratory incident and near miss information. EHSS will work with the principal investigator, supervisor and/or involved individual(s) to conduct a post incident or near miss evaluation  and identify steps and recommendations to prevent  reoccurrence.

Hazard Evaluations 

EHSS’ request for service form can be used to report a safety concern or request a hazard evaluation.
